The ladders start in very different places. Easy opens at 1.01x, 1.02x, 1.05x, 1.09x, 1.13x, 1.18x. Medium opens at 1.06x, 1.20x, 1.36x, 1.54x, 1.76x, 2.03x. Same screen, same button, two completely different sessions.
The bonus costs one hundred stakes, always. Whatever difficulty card you press, the price is 100× the bet in the box: 500 at a bet of 5, 20,000 at the maximum bet of 200.
